Beginning with final grades for the Fall 2003 semester, the Office of the Registrar will no longer mail a paper copy of grades, unless a copy is requested. Students may view their grades on-line via insideND at http://inside.nd.edu/ . If a student requires a printed grade report, he/she may request a copy using this form. This request is valid for one academic year (Fall through Summer). Please fill out this form prior to final exam periods.

The Printed Grade Report includes the current semester grades. No Social Security Number is displayed on this document. Academic Honors are printed on this document.

Undergraduate: Grades will be mailed to the student’s permanent home address.

Graduate: Grades will be mailed to the local address on record.

If you require an additional grade report be sent to a different address, please provide that address here. This section applies ONLY to ADDITIONAL GRADE REPORTS. Additional grade reports should ONLY be used for personal use or for a financial sponsor. The purpose of this report in not for scholarship, admission, or verification information.
