TRANSCRIPT REQUEST
Page 2:
Social Security Number/Date of Birth - Your Social Security Number
and Date of Birth are considered private. The Office of the Registrar no longer lists the completed Social Security number or year of birth on a transcript. If available, the Social Security Number and Date of Birth are listed
on your transcript as follows:
Student Number: XXX-XX-0123
Date of Birth: 01-01-XXXX
Processing Time - Transcript orders are processed as quickly as possible and normally leave the Office of the Registrar within two days of receiving the order. At the end of the semester, orders may require more time due to the volume of requests normally received.
Miscellaneous Information -
The University issues academic transcripts only for academic work
completed at Notre Dame. Requests for transcripts of courses taken
at other institutions must be directed to those institutions.
Evaluative information such as courses completed, grades
earned, etc. cannot be issued to an outside party unless the student/former student
has given the Registrar written permission to release this information
to that party. The Registrar may, however, disclose "directory
information" as allowed under The Family Education Rights & Privacy
Act (FERPA) without special permission. A list of the directory information can be reviewed at The
Family Educational Rights & Privacy Act web page.
Effective March 1, 2000 the University of Notre Dame discontinued
embossing the University seal on its transcripts. This procedure has
been replaced by using secure transcript paper with the preprinted light
blue University seal overlaid by the University Registrar's signature
in white.
Grading scale
and calculation information can be reviewed on the Office of the Registrar website. Grading scale information prior to 1988 is also available.
Questions and comments regarding transcripts can be e-mailed to the Office
of the Registrar at transcripts.1@nd.edu or by calling (574)631-7043.
