Club Sports Certification

When a Club Sport association requires enrollment and/or academic record certification, this can be obtained through the Office of the Registrar using the process below. Completion of the certifications may need information from other offices and will require ample notice. Time for completion will be approximately 5 business days.

The following must be completed for each event:

• Club sponsor or representative sends any required verification form from their Club Sport association to ClubSportVerify@nd.edu. This form must contain all required information and be saved in PDF or Excel format. This email must include event name and club sport, date of event, due date of the form,  and where the form needs to be sent after completion.

• Each participant must complete the Club Sports FERPA Release Approval form. This form must be completed for each event. Any student who does not complete the Club Sport Release form two business days prior to the form due date can not be certified to participate and will be removed from the verification form.
